Transition
Changing education settings can be daunting for any child or young person, as they prepare to leave behind everything they know and feel comfortable with to face the unknown. Transitions can be particularly challenging for children who experience attachment, trauma, or other additional needs. It’s therefore important to plan and prepare effectively to achieve a successful transition for our children and young people.
These transition plans should be reflected within the PEP for the children in our care, and should be clearly visible within other support plans for those children not in our care but supported by our other services (such as Child in Need, Child Protection or Youth Justice plans).
